To Register, Purchase this plan on my online store and download the "New Client" paperwork attached to the download. Once you have completed the paperwork you can email it back to me at jake.fitview@live.com. From there , I will review your questionnaire and then contact you with A time to do your initial call -in with me and discuss your goals. 

*This service is A monthly service, I only take online coaching clients who are willing to make A 90-day or more commitment. The service rate for this product will be automatically charged on the same day monthly. Your account will be charged once you complete registration, Our team will notify you with A receipt via email and all charges will be discussed in the initial consultation and strategy session
